Program & Participation
How does the ZealCare program work?
Each participant is paired with an expert health coach trained in chronic-condition-specific behavior change. Participants are supported through virtual sessions, group visits with other participants, and personalized goal setting. We focus on empowering people to take charge of their health with tools and resources that fit into daily life.
What conditions does ZealCare focus on?

How long does the program last?
Most participants engage for about 12 months, with options for continued coaching and support to maintain progress. The program is responsive to participants’ needs.
